29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question.   On or before October 7, 2019, the employer did not provide training to employees during MCWPs descent operations. Employer did not ensure the cylinder hooks and the secondary hooks were properly engaged on the mast rung before raising or lowering the platform.
