29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) The employer is failing to ensure that all employees exposed to fall hazards while working on a commercial roof are utilizing fall protection. This most recently documented on October 07, 2019 on the jobsite located at 15230 W Maple Rd, Omaha, NE 68116. Six employees were tearing off and installing shingles on the roof of an existing commercial building and were not utilizing any form of fall protection. The building has an approximate roof pitch of eight in twelve and ground to eve height is approximately thirteen feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) The employer is failing to ensure that all employees are protected from fall hazards when entering and exiting the landing surface of a commercial roof. This was most recently documented on October 07, 2019 on the jobsite located at 15230 W Maple Rd, Omaha, NE 68116. Six employees were using an aluminum extension ladder to enter and exit the roof where the side rails were not extended to at least 3 feet above the landing surface which is about thirteen feet above the ground.
