29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  (a)  U.S. Forest Service at 902 SE North Bend Way, North Bend, Washington-- Construction and Maintenance Bay;  An unmarked grinder (only grinder in the area) had the tool rest approximately ¼ inch from the grinding wheel.   (b)  U.S. Forest Service at 902 SE North Bend Way, North Bend, Washington-- Carpenter Shop:  A grinder (forest service no 060505-003864) had both the left side and right side tool rests approximately ¼ inch from the grinding wheel.    This exposes employees to possible impalement hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a)  U.S. Forest Service at 902 SE North Bend Way, North Bend, Washington-- Recreation Office;  A Panasonic microwave was plugged into a surge protector (relocatable power tap). The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.     The U.S. Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1067722, citation number 1, item number  and was affirmed as a final order on October 5, 2015, with respect to a workplace located at Amboy, Washington.  The U.S. Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1067233, citation number 1, item number 3 and was affirmed as a final order on October 15, 2015, with respect to a workplace located at Vancouver, Washington.
