Employee working from a makeshift scaffold platform (pallet), supported by a JCB all-terrain forklift, serial # JCB5CG7GPK2911143 was exposed to fall hazards in that the employer did not have manufacturer's prior written approval to use the platform with the forklift to lift personnel.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: Employee was exposed to fall hazards. Employee engaged in roofing activities was exposed to fall hazards of up to 25 feet from the scaffold platform (pallet) to the ground below in that the platform was not secured to the forks of the all terrain forklift which was supporting the platform.
