OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and/or amputation hazards:    a) Warehouse Dock A, Door #2: The access gate to the horizontal compactor/baler with an inoperable interlock to prevent access to the moving ram during operation, as observed on or about November 5, 2019. 29 CFR 1910.178(m)(4): Industrial truck drivers were not prohibited from placing extremities outside the running lines of the truck.    a) Main Travel Lane:  Employees were exposed to crushing hazards while operating a Crown stand-up powered industrial truck. On or about October 30, 2019, an employee leaned out of the operator's compartment and was pinned in between the truck and a structural column, resulting in a fatal crushing injury.    Abatement Certification and Documentation Required.
