29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:  a)  For an employee who suffered a fingertip amputation on October 25, 2019.  The employer reported the amputation on October 28, 2019.  a) On October 25, 2019, at the production floor, an employee suffered a work-related injury that resulted in a left thumb tip amputation. The employer was aware of the amputation on the evening of October 25, 2019. The employer notified OSHA on October 28, 2019 at 10:12 AM of the amputation incident.
