29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:        Excavation site, Potranco and Arcadia Ridge.  On or about November 05, 2019 and at times prior thereto, employees were exposed to a struck-by and caught-in hazard while working in a trench , and with spoil piles less than 2 feet from the edge.

On or about November 05, 2019 and at times prior thereto, employees were exposed to a caught-in hazard while taking measurements in a trench, with no designed support system, shielding system, or other protective system installed.
