Approximately four conduit junction boxes/panels had openings that were not closed, exposing employees to electric shock, on or about, November 12, 2019.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.305(g)(2)(ii): Flexible cords were not used only in continuous lengths without splice or tap:  (a): Alpha section C Bag Machines - The flexible cord used to power the motor in the bag machine was not used in a continuous manner. The flexible cord contained a splice that was covered by electrical tape, exposing employees to electric shock, on or about, November 12, 2019.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a): Alpha, Section C Bag Machines-  One or more methods of machine guarding was not provided to protect the operator from hazards created from the rotating draw rolls. Employees adjust the cardboard placed directly above the draw rolls. An employee suffered a finger amputation when he was adjusting the cardboard and his finger was caught in the ingoing nip point created by the draw rolls, on or about, November 11, 2019.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).  Omega Plastics Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 1405285, citation number 1, item number 1 and was affirmed as a final order on November 4, 2019 , with respect to a workplace located at Page and Schuyler Avenues in Lyndhurst, NJ .
