(a) Northwest- Fanuc Drill Mate; C-Model 10: On or about November 22, 2019, the employer did not ensure barrier guarding, interlock and/or light curtains were properly implemented as part of the integrated safety system. This allowed employee(s) to open the door that accesses the point of operation in an energized state, exposing the employee(s) to ingoing nip point, crushing hazard and amputation injury. (b) Northcentral- Fanuc Drill Mate; C-Model 10: On or about November 22, 2019, the employer did not ensure barrier guarding, interlock and/or light curtains were properly implemented as part of the integrated safety system. Bliss - C150 Mechanical Press: On or about November 22, 2019, the employer did not establish and provide a die setting procedure to employees conducting die changes on the press, to insure any procedure being followed met the requirements of paragraph (c) of this section. The Winco Stamping, Inc. company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.217(d)(9)(i) , which was contained in OSHA inspection number #1341410, citation number 1, item number 5, and was affirmed as a final order on November 27, 2018, with respect to a workplace located at 650 Precision Court, Miamisburg, OH 45342.
