29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     a.  On or about October 28th 2019, at 7000 NW 60th Street, Miami FL 33166, an employee was exposed to an amputation hazard while working with a table saw that had not been equipped with a guard.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:    a.  On or about October 28th 2019, at, 7000 NW 60th St. Miami, FL 33166 the employer did not report to OSHA within 24 hours the hospitalization of an employee after an employee received an amputation injury with a table saw.  The employer reported the incident to OSHA on November 14, 2019.
