29 CFR 1926.451(g)(1)(iv): Each employee on a self-contained adjustable scaffold was not protected by a guardrail system.    a. 15 Lump Gulch Rd, Clancy MT, 59634: On November 19, 2019 and at times prior, employees were exposed to fall hazards while measuring rafters from a GENIE GS-2632 Scissor Lift (#14v02864) at a height of 13 feet, without the entry gate fully closed and latched.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.     a. 15 Lump Gulch Rd, Clancy MT, 59634: On November 15, 2019, and at times prior the employer failed to report the in-patient hospitalization of an employee within 24-hours a work-related incident. An employee sustained injuries requiring hospitalization on November 13, 2019 at approximately 1:30 P.M. The employer notified OSHA on November 15, 2019 at approximately 7:00 P.M.
