29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    On November 25, 2019, located on the north side of a commercial exterior structure, an employee was working from an 8' stepladder in the closed position, as there were no frequent and/or regular inspections performed by a competent person to ensure ladders were installed and utilized safe manner.

On November 25, 2019,  located on the northeast side of an exterior commercial structure, an employee was observed working from a Werner 8' fiberglass stepladder that was leaning against the exterior structure while in the closed position for which it was not designed for this purpose.
