29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   On or about 4 December, 2019, at the above addressed job site in the front of the building, an employee was working from an awning deck without the use of fall protection, exposing the employee to a fall hazard of approximately 11 feet.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  On or about 4 December, 2019, at the above addressed job site in the front of the building, an employee was working on a deck while standing on an A frame ladder which was in its closed position leaned up against the wall, exposing the employee to a fall hazard of approximately 11 feet.
