29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l):  Store: Employer did not have the written certifications for employees who had received training on electric pallet jack operations.   Note: The certification shall include name of operator, the date of training, the date of the evaluation, and the evaluation, and the identity of the person performing the training and/or evaluation.
