29 CFR 1910.178(n)(4): Industrial truck driver(s) were not required to travel with the load trailing whenever the load obstructed forward view nor were industrial truck driver(s) required to slow down and sound the horn at cross aisles and wherever vision was obstructed:  On or about October 28, 2019, the employer failed to ensure that industrial truck driver(s) traveled in reverse when their load obstructed their view and the employer failed to ensure that industrial truck driver(s) sounded their horn when their load obstructed their view. A PIT operator operating a Raymond Reach Truck (model: EASIR40TT, S/N: EZ-D-09-39855) was involved in an accident in D Freezer Aisle 26.

29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics was not provided to the operator when the operator was involved in an accident or near-miss incident:  On or about October 28, 2019, the employer failed to retrain the powered industrial truck (PIT) operator who was involved in an accident, before allowing the operator to begin PIT operation again. A PIT operator operating a Raymond Reach Truck (model: EASIR40TT, S/N: EZ-D-09-39855) was involved in an accident in D Freezer Aisle 26.  The Great Lakes Cold Storage Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(4)(i), which was contained in OSHA inspection number 1322823, citation number 1, item number 2 and was affirmed as a final order on 8/13/2018, with respect to a workplace located at 6531 Cochran Rd. Solon, OH 44139.
