29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    On or about December 17, 2019, the employer did not ensure that employees assigned to perform un-jamming, cleaning, and other servicing tasks on the PTR Baler Compactor were required to apply energy controls in the following sequence:    1) turn off and shut down the machines,  2) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s),  3) affix lockout or tagout devices to each energy isolating device,  4) verify that isolation and deenergization of the machines or equipment have been accomplished.    Employees were exposed to caught-in hazards likely to result in amputation of arms, hands and fingers.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(2): General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself.    On about December 17, 2019, the employer did not ensure that the guarding affixed to the PTR Baler Compactor's gate was free from damage that could cause injury.  The guard had multiple sharp edges that exposed employees to laceration hazards when opening, closing and handling the baler gate.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

29 CFR 1910.303(b)(7)(iv): There shall be no damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.    On or about December 17, 2019, the employer did not ensure that the PTR Baler Compactor was free from damaged parts that could affect the safe operation the equipment.  The black power switch on the control cabinet was broken and inoperable, exposing employees to electrical and crushing hazards.      Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings shall be effectively closed to afford protection substantially equivalent to the wall of the equipment.  On or about December 17, 2019, the employer did not ensure that electrical equipment was effectively closed with one missing knockout on Westinghouse unit #9, 480 volt heater electrical box located at the PTR baler compactor machine.   Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

29 CFR 1910.303(g)(1)(ii): Working space required by this standard may not be used for storage. When normally enclosed live parts are exposed for inspection or servicing, the working space, if in a passageway or general open space, shall be suitably guarded.  On or about December 17, 2019, the employer failed to provide sufficient access and working space about electrical equipment next the PTR Baler Compactor when the areas were blocked by excess cardboard and metal wire cable.   Abatement certification is required for this item in accordance with the requirements of 1903.19(c).
