The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    On or about December 17, 2019, the employer did not ensure that employees assigned to perform un-jamming, cleaning, and other servicing tasks on the PTR Baler Compactor were required to apply energy controls in the following sequence:    1) turn off and shut down the machines,  2) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s),  3) affix lockout or tagout devices to each energy isolating device,  4) verify that isolation and deenergization of the machines or equipment have been accomplished.    Employees were exposed to caught-in hazards likely to result in amputation of arms, hands and fingers.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
