29 CFR 1926.97(c)(2)(ii): Insulating equipment was not inspected for damage before each day's use and immediately following any incident that could reasonably be suspected of causing damage:  On January 15, 2020, at the jobsite, employees elevated in a bucket truck were not protected from electric shock and electrocution hazards due to protective devices including but not limited to rubber blankets and guts not being inspected by a competent person before each day's use to ascertain if any deficiencies such as pinholes or tears existed.

29 CFR 1926.960(c)(2)(ii)(B): The employer did not ensure that the employee does not remove the rubber insulating gloves and sleeves until he or she is in a position where he or she cannot reach into the minimum approach distance, established by the employer under paragraph (c)(1) of this section:  On January 15, 2020, at the jobsite, the employer did not ensure that employee(s) who were performing ground work while walking a guy wire to an anchor point where live-line work was ongoing did not remove rubber insulating gloves until they were in a position where they could not reach into the minimum approach distance (MAD).   Arkansas Electric Cooperatives, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.960(c)(2)(ii)(B), which was contained in OSHA inspection number 1118677, citation number 1, item number 2 and was affirmed as a final order on 04/20/2016, with respect to a workplace located at Fairgrounds Rd & E 32nd Streets, Stillwater, Oklahoma.
