29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:     (a)  On or about January 8, 2020, Lokal Communities, LLC allowed the work area to be disorganized and unkept, exposing trade employees to punctures from protruding nails and slips, trips and fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a)  On or about January 8, 2020, Lokal Communities, LLC, as a controlling employer, did not ensure that Centerline Builders and their subcontractors protected their employees from falls in excess of six feet, while they were in the process of constructing a residential building.
