29 CFR 1915.71(b)(4): All lumber used in the construction of scaffolds shall be sound, straight-grained, free from cross grain, shakes and large, loose or dead knots. It shall also be free from dry rot, large checks, worm holes or other defects which impair its strength or durability.    Railway #4, M/V San Pedro:     Employees were conducting welding activities around the exterior of the vessel using a Safeway Scaffolding System for access.  The employer did not ensure scaffolding planks that contained cracks, notches, splits, delamination and holes were removed from service. Employees were exposed to the potential of plank failure, and fall hazard(s).    Al Larson Boat Shop was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1915.71 (b)(4), which was contained in OSHA inspection number 1245274, citation number: 1, item number: 1 and was affirmed as a final order on 08/24/2017, with respect to a workplace located at 1046 S. Seaside Avenue San Pedro, CA. 90731.

Railway #4, M/V San Pedro:     Employees were conducting welding activities around the exterior of the vessel using a Safeway Scaffolding System for access.  The employer did not ensure scaffolding guardrail(s) top, mid and/or cross rails that were damaged/bent were removed from service. Employees were exposed to the potential of railing failure, and fall hazard(s).

29 CFR 1910.254(b)(4)(iv): Terminals for welding leads were not protected from accidental contact by personnel or metal objects:    Railway #4, M/V San Pedro:    Employees were conducting welding activities around the exterior of the vessel and internal tanks using a Miller Mark VIII 2 Arc Welder.  The employer did not ensure Power Output Insulator Cover(s) were in place to prevent accidental contact with terminals.  Employees were exposed to shock, and electrocution hazards.
