29 CFR 1910.134(k)(5)(ii): Retraining was not conducted when inadequacies in the employee's knowledge or use of the respirator indicated that the employee did not retain the necessary training or skill: M/V ICON: Railway 1: Employer required an elastomeric tight fitting respirator during welding and grinding operations and had not conducted annual retraining for employees that displayed inadequacies in knowledge and use of respirator such as recognition of not having a 'tight' fit for face seal, cleaning process, maintenance, and storage requirements. Employees were exposed to inhalation hazards.

29 CFR 1910.134(h)(1)(i): Respirators issued for the exclusive use of an employee were not cleaned and disinfected as often as necessary to be maintained in a sanitary condition: M/V ICON: on Railway 1: A 3M 6300 elastomeric respirator with cartridges used by employee during welding and grinding operations was not stored during operations in an area free from contaminants and the employee did not clean the respirator prior to reuse. Employees were exposed to ingestion and dermal hazards.
