29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: On or about 1/30/2020, at the jobsite located at 4700 State Road 7, Hollywood, FL 33019. Two employees were exposed to a fall hazard of 8 feet or more feet to a lower level while assessing a roof using an opened self-supporting ladder (A-frame type, Werner, 10') without being secured.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about 1/30/2020, at the jobsite located at 4700 State Road 7, Hollywood, FL 33019. Two employees were observed exposed to a fall hazard of up to 33'-4" while performing roofing work on a low slope roof without being protected from falling. Green Roofing & Waterproofing Technologies LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.501(b)(10), which was contained in OSHA inspection number 1103186, citation number 2, item number 1 and was affirmed as a final order on 12/15/2015, with respect to a workplace located at 2031 SW Gatlin Boulevard Port Saint Lucie, FL 34953.
