29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  On or about February 5, 2020, at 4710 SW 8 Street, Davie, Florida, an employee engaged in commercial roofing operations was exposed to electrical shocks when working in close proximity to energized overhead power lines.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about January 23, 2020, at 4710 SW 8 Street, Davie, Florida, an employee engaged in commercial roofing operations was exposed to a 13-foot fall hazard without means of fall protection.
