29 CFR 1910.23(c)(9): Portable ladders used on slippery surfaces were not secured and stabilized:    (a) Bottom of the stairway leading to the offices:  On or about 2/6/2020, an employee was working from an unsecured 40 foot extension ladder, to clean mold and stains from the walls, when the ladder slipped on the wet floor causing the employee to fall approximately 15 feet to the concrete floor.
