29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them.  At the time of the inspection, the machine specific procedures for equipment such as but not limited to the Line #5 Tuber machine did not include steps for the release from lockout or tagout before energy is restored to the equipment.   Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  The following machine guarding hazards were found;  a) At the time of the injury on 1/31/2020, the knife that cuts the tape on the Tuber machine was not guarded to prevent employee from contacting the blade and receiving an amputation injury.  b) At the time of the inspection, the guard added to the Tuber to prevent access to the cutting blade was easily removable.   c) At the time of the inspection, the portable stanchion guard was easily removed from the front of the Tuber machine exposing employees to the rotating Star wheel which caused a caught in hazard.   Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.
