29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a) Scott line, Fabrication Department: On or about February 3, 2020, and at times prior, the employer exposed employees to struck-by and/or caught-between hazards in that, employees failed to document or utilize procedures for the control of hazardous energy for hydraulic Press 209 while employees were clearing the machine and the press was in a condition where its ram drifted down when the interlock door was open.

29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them. a) Scott line, Fabrication Department: On or about February 11th, 2020, and at times prior, the employer exposed employees to struck-by and/or caught-between hazards in that, there was a change in the procedure to lockout hydraulic press 209 which required two people to perform. The machine specific procedures did not reflect this change and therefore did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them.
