29 CFR 1915.508(e)(1)(iv): The fire watch employee was not trained annually: USS Lake Champlain, Sonar 1: Employees performing hot work operations, such as but not limited to welding during ship repair operations. The fire watch employee lacked knowledge on the requirements of fire watch duty and was not trained annually. Employees were exposed to contact with fire and smoke inhalation hazards.
