29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  Location: 401 Rt. 10, Ledgewood, NJ.  a) Employer did not ensure that an employee working/walking on a roof was protected from fall hazards. Employee fell through an unprotected hole approximately 15 ft. to the ground below causing serious injuries; on or about 2/18/20.
