Body belts, harnesses, and other components used in personal fall arrest systems, work positioning systems, and travel restraint systems did not meet the requirements of 29 CFR 1910.140.    a) Warehouse - Interior, South Row 36- Hi Lo Order Picker- (Serial Number) 540 - 12- C13715 - An employee was exposed to fall hazards through an inadequate fall arrest system that did not have a form of deceleration, on or about March 2, 2020.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).      b) Warehouse - Interior,South Row 41- Hi Lo Order Picker- (Serial Number) EAS -01- AL29648 - An employee was exposed to fall hazards through an inadequate fall arrest system that did not have a form of deceleration, on or about March 2, 2020.        Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

Defective or damaged personal protective equipment shall not be used.    a) Warehouse - Interior, South Row 36- Hi Lo Order Picker- (Serial Number) 540 - 12- C13715 - An employee was exposed to fall hazards through an inadequate fall arrest system that had a damaged connector. The carabiner connecting the harness to the lanyard had a damaged gate that did not automatically close, on or about March 2, 2020.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Operators did not complete practical training and operator evaluation.  Employees were operating powered industrial trucks without required training, on or about March 2, 2020. 29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l):  (a): Facility - The employer did not certify that each powered industrial truck operator was properly trained. Employees operating forklifts did not have certification that included the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation, on or about March 2, 2020.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
