Main Street, Albia, Iowa, the employee in charge did not provide safety briefs before the workers started their work at the site.  The Bucket Operators, the employee in charge, do not provide the job briefs according to the tree trimming standards.

Main Street, Albia, Iowa, the employer had never conducted and put into writing a workplace hazard assessment.  A representative of the employer stated that no such certification exists.

Main Street, Albia, Iowa, the employer did not ensure that the Bucket Operator at 6957 Weldon Spring, MO, had donned the personal fall arrest harness properly.  The leg straps were not engaged while in an elevated aerial lift bucket.  An incident occurred with the victim 20  25 feet above ground level.

Main Street, Albia, Iowa, the employer did not ensure that the Bucket Operator at 6957 Weldon Spring, MO, had donned the personal fall arrest harness properly.  The leg straps were not engaged while in an elevated aerial lift bucket.  The importance of donning the harness properly with the leg straps had been ignored or forgotten since in an aerial bucket.  An incident occurred with the victim 20  25 feet above ground level and was tossed out of the aerial lifts bucket.

29 CFR 1904.33(a): The employer did not maintain the OSHA 300 log, the privacy case list, the annual summary  and the OSHA 301 Inident Report forms for five (5) years following the end of the calendar year that those records covered:   On or about 03/10/2020, Toms Tree Service LLC, located at 1313 S. Main Street, Albia, Iowa, the employer did not maintain their OSHA Logs for the years 2017, 2016 and 2015.  The employer had logs for 2018 and 2019, with the 2020 OSHA 300 Year-To-Date log.
