29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were  made without the manufacturer's written approval.  The capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly:    (a) Precision Building System at 10245 Yampa Ct., Commerce City, CO 80022:  On or before January 22, 2020, the controlling, creating, and correcting employer provided an Extreme all-terrain forklift and a JLG man-lift attachment to subcontractors for use on the jobsite. The forklift manufacturer's written approval of using the man-lift attachment was not obtained nor were capacity, operation, and maintenance instruction plates changed and/or added.  Subcontractor employees used the forklift with the man-lift attachment to work at elevated heights. This condition exposed subcontractor employees to fall and struck-by hazards.

29 CFR 1926.602(c)(1)(viii)(A): The safety platform was not firmly secured to the lifting carriage and/or forks when lifting personnel:  (a)   Precision Building System at 10245 Yampa Ct., Commerce City, CO 80022:  On or before January 22, 2020, the controlling, creating, and correcting employer provided an Extreme all-terrain forklift and a JLG man-lift attachment to subcontractors for use on the jobsite.  Subcontractors operated the equipment to lift employees without securely attaching the man-lift platform to the fork assembly.  The manufacturer supplied attachment hardware was not utilized.  This condition exposed subcontractor employees to fall and struck-by hazards.
