29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    Employees were exposed to struck-by hazards. Employees engaged in residential framing activities with the use of a pneumatic nail gun and a circular saw were exposed to struck-by hazards. Employees were not provided with eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At the residential construction site located at 12811-13 E Timberlake in Wichita KS;  Employees engaged in framing activities, were exposed to fall hazards of approximately 16 feet from the edge of the structure the ground below in that fall protection was not used.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  Employees were exposed to fall hazards.  Employees engaged in framing activities, using a Werner 8 foot A-frame stepladder, were exposed to fall hazards of approximately 8 feet in that the ladder was not used for the purpose that it was designed.  The ladder was used in a folded position, not in the open position with spreader bars locked.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  Employees engaged in residential framing activities were exposed to falls from approximately 16 feet, in that the aluminum ladder used to access the working level of the structure did not extend at least 3 feet above the landing surface.
