29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    1 South Spencer Rd, Spencer, MA: On Mar 17, 2020 and for twenty four months prior, the employer had not performed the annual periodic inspection that ensured each procedure was inspected for inaccuracies during an actual energy control task.  Additionally, the employer did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under each energy control procedure they were authorized to use.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  1 South Spencer Rd, Spencer, MA  On March 16, 2020 and at times prior, employees who operated the Breakdown Slitter machine #3S18 in the reverse mode were exposed to the unguarded nip point between the product and the rotating roll receiving the product.   On March 9, 2020, an employee operating that machine received hand and arm injuries in that unguarded nip point.
