29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On March 25, 2020; employees were exposed to falls of approximately 11 feet to the concrete floor while using a folded A-Frame ladder to access the second floor landing. The ladder was leaning against studs adjacent to and bellow the second floor landing. (b) On March 25, 2020; employees were exposed to falls of approximately 11 feet to the second floor landing while using a folded A-Frame ladder to access the second floor roof. The ladder was leaning adjacent to and bellow the second floor roof. Note: Abatement certification is required for this item.
