29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  On or about May 14, 2020, at Lot Number 8, Moss Park Boulevard, Orlando, Florida, 32832, employees were installing roof decking atop a 5:12 pitch roof, while wearing a Personal Fall Arrest System (PFAS), wherein the system(s) were rigged such that during a fall, the employees would have free fallen more than 6 feet and / or the system(s) were rigged such that the employee would have contacted the lower level; therefore exposing the employees to a fall hazard.   NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about May 14, 2020, at Lot Number 8, Moss Park Boulevard, Orlando, Florida, 32832, employees were installing roof decking atop a 5:12 pitch roof, while wearing a Personal Fall Arrest System (PFAS), during which the employer did not provide a fall protection training program, so as to enable the employees to recognize the hazards of falling and the procedure to minimize these hazards, nor did the employer ensure that a Competent person supervised, implemented and monitored the fall protection program; therefore exposing the employees to a fall hazard.   NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.  On or about May 14, 2020, at Lot Number 8, Moss Park Boulevard, Orlando, Florida, 32832, employees were installing roof decking atop a 5:12 pitch roof, while wearing / using a Personal Fall Arrest System (PFAS), wherein the employer did not ensure that each employee had been trained, by a Competent person qualified in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, and other protection to be used, in subject areas such as PFAS limitations / minimum fall height, nor did the employer ensure that a Competent and Qualified person evaluated the fall protection knowledge and skills of trainees through written testing and performance assessments; therefore exposing the employees to a fall hazard.  NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
