Employees were exposed to struck-by and caught-in-between hazards that could result in serious injuries while performing this task.

Employees did not de-energize and lockout all hazardous energy sources of the Milacron Maxima Plastic Injection Press # 91 (Product # MPS-1100; Serial # M7001AB010099) prior to conducting activities such as, but not limited to, troubleshooting air poppets in the press. Employees were exposed to struck-by and caught-in-between hazards that could result in serious injuries while performing this task.
