29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:    a) On or about June 6, 2020, the employer did not ensure that employees required to operate powered industrial trucks, including, but not limited to the CombiLift CB8 - Model C9000CB - Serial Number 21139 forklift. Employees were not   re-evaluated at least once every three years, thereby exposing employees to struck-by hazards.
