29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   On or about June 17, 2020, during the installation of roofing material at the job site located at 4735 Massillon Rd, N. Canton, Ohio 44720; the employer did not ensure employees working on the roof of a commercial location were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   On or about June 17, 2020, during the installation of roofing material at a job site located at 4735 Massillon Rd, N. Canton, Ohio 44720; the employer did not ensure employees working on the roof of a commercial location were protected from a fall of approximately 11.2 feet by utilizing a fall protection system.

Canton, Ohio 44720; during the installation of roofing material on a commercial roof, the employer did not ensure portable ladders extend 3 feet above the upper landing surface.
