29 CFR 1910.147(c)(2)(ii): The employer's tagout energy control program procedures did not successfully comply with 29 CFR 1910.147(c)(3) and no lockout was provided for employee protection: a) Hoppers for the dust collector systems: March 30, 2020 - In dust collector system hoppers, employees were exposed to a crush-by hazard while performing maintenance when the site and contract employer failed to ensure the "tags only" energy isolation procedure also included a "tags plus" isolating device to ensure the tags only procedure provided employee protection equivalent to a lock out and tag procedure.

29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and maintenance was performed by a crew, craft, department or other group: a) Hoppers for the dust collector systems: On or about March 30, 2020 - In dust collector system hoppers, employees were exposed to a crush-by hazard while performing maintenance in dust collector hoppers when the site and contract employer failed to implement the group tagout personal accountability procedure by having each employee sign-on and sign-off the Master Red Tag from before restarting a dust hopper.
