29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: (a) On or about 6/16/2020 - Argon Oxygen Decarburization (AOD) Area, employees were exposed to a fall hazard of approximately 12 feet 3 inches when the guardrail system was missing the midrail at the platform opening where employees stand to perform tasks such as, but not limited to, adding materials to the vessel, sampling, taking temperatures, visual inspections, and oxygen lancing at the AOD vessel opening.

(a) On or about 6/16/2020 - Argon Oxygen Decarburization (AOD) Area, employees were exposed to burn hazards when the guardrail system was missing the midrail at the platform opening where employees stand to perform tasks such as, but not limited to, adding materials to the vessel, sampling, taking temperatures, visual inspections, and oxygen lancing at the AOD vessel opening.
