Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by hazards. (a) The radio controlled wireless pendant was not turned off while rigging was being completed, exposing employees to a crushing hazard. Among other methods, feasible and acceptable abatement methods to correct the hazard include turning off the pendant when the pendant is not in use during the rigging process and verifying the pendant has been turned off; or placing a sliding cover over the controls to ensure the buttons aren't inadvertently activated."
