29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     Worksite located at 900 Swift St., N. Kansas City, Missouri- Employee was exposed to a fall hazard in that the ladder being used to access the upper landing surface was not extended at least 3 feet above the upper landing surface.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:     Worksite located at 900 Swift Ave, N. Kansas City, Missouri- Employees were observed working and walking on the edges of the roof without the use of any means of fall protection.    The Bielak Roofing Company  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1387822, citation number 1, item number 1,  and was affirmed as a final order on 6/27/2019, with respect to a workplace located at 3205 Main St., Kansas City, Missouri.    The Bielak Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1274239, citation number 1, item number 1,  and was affirmed as a final order on 1/3/2018, with respect to a workplace located at 633 E. 63rd St., Kansas City, Missouri 64112
