29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about June 21, 2020, the employer did not ensure employees performing framing activities were trained in recognition and procedures needed to minimize fall hazards. This condition exposed employees to fall hazards greater than 6 feet to the ground below.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On or about July 21, 2020,  the employer did not ensure that employees were protected from falls from heights, while framing for window openings. This condition exposed employees to a greater than 6-foot fall to the ground below.   (b)  On or about July 27, 2020, the employer did not ensure that employees were protected from falls from heights, while engaged in framing activities. This condition exposed employees to a greater than 6-foot fall to the ground below.   Rio Verde Framing, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1358220, citation number 1, item number 1 and was affirmed as a final order on February 26, 2019, with respect to a workplace located at 1008 N. Preserve Ct, Wichita Kansas.

29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours.  On or about July 21, 2020, the employer did not report an employee's work-related inpatient hospitalization to OSHA within twenty- four (24) hours.  The employee sustained multiple injuries from a fall from elevation.  The employer reported the hospitalization on July 23, 2020.
