29 CFR 1926.453(a)(2): Field modification of the aerial lift was not certified in writing by the manufacturer or by any other equivalent entity, such as a nationally recognized testing laboratory, to be in conformity with all applicable provisions of ANSI A92.2-1969 and this section and to be at least as safe as the equipment was before modification:  a. On or about 7/28/2020 at 3100 Zingara Road Conyers, GA where the employer allowed for the attaching of unauthorized attachments to the lift in order to lift sheets of drywall and other materials external to the basket without written approval from the manufacturer or Sunbelt rental company exposing workers to a tipping and struck-by hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about 7/28/2020 at 3100 Zingara Road Conyers, GA, on the rooftop where the employer failed to ensure that workers were protected from fall hazards above 20 feet where a worker was observed at the edge without fall protection.
