29 CFR 1926.961(c)(13): Reenergizing lines and equipment. The employer did not ensure that no one initiated action to reenergize the lines or equipment at a point of disconnection until all protective grounds have been removed, all crews working on the lines or equipment release their clearances, all employees are clear of the lines and equipment, and all protective tags are removed from that point of disconnection:  (a) On or about July 30, 2020 employees were exposed to arc flash/blast hazards when protective grounds were not removed prior to re-energizing the lines.
