29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a) Best Roofing Restoration, LLC at 4540 N Argonne Street, Denver, CO 80249:  On April 14, 2020 the employer did not ensure that employees were protected from injury, in that four employees installing shingles on a two-story residential home, with a roof pitch of approximately 4-in-12, were not utilizing a fall protection system.  This condition exposed four employees to a fall hazard of approximately 15-20 feet.
