29 CFR 1910.22(d)(1): Walking-working surfaces were not inspected, regularly and as necessary, and maintained in a safe condition.  (a)  On or about August 12, 2020, and at times prior thereto, the employer did not develop procedures to ensure proper inspections of a walking-work surface were performed prior to use.  The modular non-penetrating guardrail system on the roof had been moved and exposed employees to an approximate 28-30 foot fall hazard.    Note: Abatement certification AND supporting documentation of correction are required for this item.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling:   (a)  On or about August 12, 2020, and at times prior thereto, employees were exposed to fall hazards when they accessed the roof structure without an adequate fall protection system securely in place.  Note: Abatement certification AND supporting documentation are required for this item.
