29 CFR 1910.147(c)(6): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. The periodic inspection must:  [A] be performed by an authorized employee other than the ones utilizing the energy control procedure being inspected.    [B] be conducted to correct any deviations or inadequacies identified.  [C] where lockout is used for energy control, include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected.  [D] where tag-out is used for energy control, include a review between the inspector and each authorized and affected employee, of that employee's responsibilities under the energy control procedure being inspected, and the elements set forth in paragraph (c)(7)(ii) of this section:    On or about 8/18/20, the employer failed to conduct a periodic inspection of the lockout/tagout procedures, that includes a review between the inspector and each authorized employee. Energy sources including, but not limited to hydraulic, pneumatic, and electrical. 29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    On or about 8/18/20, the employer failed to ensure that the lockout tagout procedures included the following steps and were conducted in the following sequence:    1) Prepare for shutdown,  2) Machine or equipment shutdown,  3) Machine or equipment isolation,  4) Lockout/tagout device application,  5) Relieve/disconnect/render safe stored energy,  6) Verification of energy isolation.

29 CFR 1910.219(b)(1): Flywheel(s) with parts seven feet or less above floor(s) or platform(s) were not guarded in accordance with the requirements specified in 29 CFR 1910.219(b)(1)(i) through (b)(1)(iv)(a):    On or about 8/18/20, the employer failed to ensure that the flywheel of the Rousselle Stamping Press, model #6360, serial #10241 (#2), was at least seven feet (7') from the walking/working surface, or effectively guarded. The exposed flywheel was about 64", or about 5.3' from the walking working surface.

29 CFR 1910.219(f)(1): Gear(s) were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii):    On or about 8/18/20, the employer failed to ensure that the gears on the following machinery were effectively guarded by complete enclosure:    a) Bliss power press, serial # HP46426 H60532 1968 (#10), and  b) Bliss power press, serial # H 61244 HP 46928 1969 (#11).    The exposed gears were about 75" from the walking/working surface, and about 36" from the operator controls.
