29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles. On the east side of the construction site, employees engaged in cornice work used pneumatic nail guns and were not wearing safety glasses. The employees were exposed to struck-by hazards and eye injuries.

29 CFR 1926.304(d): Each portable, power-driven circular saw withdrawn from the work did not automatically and instantly return the lower guard to the covering position of the saw. On the east side of the construction site, employees operating Makita skill saws with the lower guards wedged in the up position while cutting cornice material were exposed to amputation hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed On the east side of the construction site, employees using an extension ladder that did not have the side rails extended at least 3 feet over the edge of the working platform were exposed to fall hazards of greater than 6 feet.
