29 CFR 1926.502(b)(1): The top edge height of the top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level: (a) Kirk Bridge: On September 9, 2020, an employee was working on top of the bridge abutment taking a measurement. The horizontal life line used as top rail to protect the employee from an 8 foot fall hazard was measured at 36 inches above the working level.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high: (a) Kirk Bridge: On September 9, 2020, an employee was working on top of the bridge abutment taking a measurement. The rebar used as an intermediate rail to protect the employee from an 8 foot fall hazard was measured at 19 inches above the working level.
