29 CFR  1910.22(b): The employer did not ensure that each walking-working surface can support the maximum intended load for that surface:  (a)  Liberty Oilfield Services Inc., at 9540 E 104th Avenue, Henderson, CO: On and preceding 9/8/2020, the employer did not ensure that the roof of the lab structure was capable of supporting equipment stored upon it and capable of supporting the weight of employees working upon it.  Equipment, including quality control and laboratory equipment, was stored upon the flat roof of the laboratory.  A portion of the laboratory roof was constructed of ceiling tiles.  An employee fell through the roof when he stepped upon the section roofed with ceiling tiles.  This condition exposed two employees to an approximate ten foot fall hazard.

29 CFR  1910.28(b)(13)(ii): When work was performed at least 6 feet (1.6 m) but less than 15 feet (4.6 m) from the roof edge, the employer did not ensure each employee was protected from falling by using a guardrail system, safety net system, travel restraint system, or personal fall arrest system:  (a)  Liberty Oilfield Services Inc., at 9540 E 104th Avenue, Henderson, CO: On 9/8/2020, the employer did not ensure that employees were protected from falling when working upon the roof of an interior structure.  Equipment, including quality control and laboratory equipment, was stored upon the flat roof of the laboratory.  Employees were directed to retrieve equipment from the roof.  A guardrail system was not in place.  Employees were not protected by a restraint system or fall arrest system.  This condition exposed two employees to an approximate ten foot fall hazard.

29 CFR  1910.23(c)(4): Ladders are used only on stable and level surfaces unless they are secured or stabilized to prevent accidental displacement;  (a)  Liberty Oilfield Services Inc., at 9540 E 104th Avenue, Henderson, CO: On 9/8/2020, the employer did not ensure that a stepladder was used only for the purpose for which it was designed.  Employees utilized an approximate fourteen foot fiberglass stepladder to ascend to the roof of an interior structure in the vehicle maintenance building.  The stepladder was leaned against the structure, rather than extending its legs.  The stepladder was not used according to its design.  This condition exposed two employees to an approximate ten foot fall hazard.
